ABSTRACT

RESUMEN Se presenta un caso de ataque isquémico transitorio con sintomatología compatible con lesión de la circulación cerebral posterior, secundario a embolia aérea iatrogénica. Se describe la evolución clínica y las consideraciones más relevantes de la atención y el diagnóstico del ataque cerebrovascular de la circulación posterior. En cuanto a la embolia gaseosa, se describen los métodos diagnósticos, las intervenciones clínicas y las opciones de tratamiento disponibles.


SUMMARY Here ia a case of transient ischemic attack with symptoms compatible with injury to the posterior cerebral circulation, secondary to iatrogenic air embolism. Clinical evolution and the most relevant aspects for the care and diagnosis of cerebrovascular stroke of the posterior circulation are described. Regarding air embolism, the diagnostic methods, clinical interventions, and available treatment options are described.

RESUMEN Introducción: el control de daños en Ortopedia es aplazar la reparación definitiva de lesiones traumáticas, con el objetivo de la recuperación fisiológica deteriorada por lesiones y/o complicaciones que pueden poner en peligro la vida, realizando entonces procedimientos quirúrgicos sencillos, como una estabilización quirúrgica externa de las fracturas. Objetivo: determinar el comportamiento del control de daños ortopédicos en politraumatizados pediátricos. Materiales y método: se realizó un estudio observacional, descriptivo, prospectivo, de corte transversal, de enero del 2015 a diciembre del 2018. El universo estuvo constituido por 22 pacientes menores de 19 años de edad, politraumatizados en el período; la muestra por 15 pacientes, a los que se les aplicó control de daños ortopédicos, según criterios de selección. Resultados: se aplicó control de daños ortopédicos a 15 infantes. El más afectado fue el sexo masculino con el 73,3 %. Prevalecieron los traumatismos de los miembros inferiores con el 58,3 % y las fracturas cerradas con un 53,8 %. La osteomielitis crónica fue la complicación que predominó, en el 20 % de la muestra. En el 73,3 % de los casos se evaluó como satisfactoria la aplicación del control de daños ortopédicos en los politraumatizados. Conclusiones: a la totalidad de los traumatizados se le aplicó control de daños ortopédicos. Predominó el sexo masculino y el grupo de edad de 9 -14 años. Los miembros inferiores aportan la mayor cantidad con el fémur y la tibia. El control de daños ortopédicos se evaluó satisfactoriamente en la mayoría de los pacientes estudiados (AU).


ABSTRAC Introduction: the control of damages in Orthopedics is to postpone the definitive repair of traumatic lesions, with the objective of the physiologic recovery deteriorated by lesions and/or complications that can put in danger the life, carrying out simple surgical procedures, like an external surgical stabilization of the fractures at that time. . Objective: to determine the behavior of the orthopedic damage control in pediatric politraumatized patients. Material and method: a cross-sectional, prospective, descriptive, observational study was carried out from January 2015 to December 2018. The universe was formed by 22 patients younger than 19 years politraumatized in the period and the sample formed 15 patients; they underwent orthopedic damage control, according to the selection criteria. Results: the orthopedic damage control, was applied to 15 children. Male sex was the most affected one. Trauma in the lower limbs with 58.3 % and closed fractures with 53.8 prevailed. The predominating complication was chronic poliomyelitis in 20 % of the sample. The orthopedic damage control in politraumatized patients was assessed as successful in 73.3 % of the cases. Conclusions: the orthopedic damage control was applied to the total of traumatized patients. The male sex and the 9-14 years-old age group predominated. Lower limbs contributed with the biggest quantity of trauma, in femur and tibia. The orthopedic damage control was assessed as successful in most of studied patients (AU).

RESUMEN Las indicaciones de la cirugía de mínimo acceso en el niño han aumentado. Lo que ha traído consigo que aumente el espectro de las entidades resueltas por esta novedosa técnica. En marzo de 2017, se comenzaron a aplicar de manera sistemática estos procedimientos en el Hospital Docente Pediátrico "Eliseo Noel Caamaño", de Matanzas. Con este artículo se pretende describir los resultados de la cirugía mínimamente invasiva en Pediatría, durante su primer año en la mencionada provincia. Se realizó un estudio descriptivo prospectivo para presentar los resultados durante el primer año. Se operaron 77 pacientes. Predominó el sexo femenino, y el grupo etario de 15-19 años. Las intervenciones más realizadas fueron la apendicectomía, 50,6 % y la colecistectomía, 28 %. Se registró un 4 % de conversiones y un 4 % de complicaciones (AU).


ABSTRACT The indications of minimal access surgery in children have increased and the spectrum of the entities resolved with this new technique has enhanced. In March 2017, these procedures began to be used systematically at the Teaching Pediatric Hospital "Eliseo Noel Caamaño", of Matanzas. In this article, the authors pretend to describe the results of the minimally invasive surgery in Pediatrics during the first year in the before-mentioned province. A descriptive prospective study was conducted to present the achieved results. 77 patients underwent surgery. Female sex predominated and the age group of 15-19 years. The most common surgeries were appendectomy (50.6 %) and cholecistectomy (28 %). A 4 % of conversions and 4 % complications were recorded (AU).

ABSTRACT

Abstract Objective This paper aims to evaluate patients with articular calcaneal fractures treated with a minimally invasive surgical technique between January 2015 and August 2016, with emphasis on radiographic results. Methods Retrospective study of 49 patients with 64 displaced calcaneal fractures treated with open reduction by minimal lateral access to the subtalar joint and minimal fixation. Pre- and postoperative radiographic studies were performed to measure the angles of Böhler and Gissane. Results The average angle of Böhler before surgery was 2.5º, increasing to an average value of 25.3º after the minimally invasive surgical treatment. The average angle of Gissane before surgery was 136.3º, decreasing to an average value of 114.3º after the procedure. Conclusion The minimally invasive surgical technique improves the radiographic parameters of intra-articular calcaneal fractures, with appropriate anatomical restoration of anatomical shape.


Resumo Objetivo Avaliar pacientes com fraturas intra-articulares de calcâneo tratados entre janeiro de 2015 e agosto de 2016 com técnica cirúrgica minimamente invasiva, com ênfase no resultado radiológico. Métodos Estudo retrospectivo de 49 pacientes com 64 fraturas intra-articulares de calcâneo, submetidos a tratamento cirúrgico minimamente invasivo. As lesões foram tratadas com redução aberta por acesso mínimo lateral à articulação subtalar e fixação mínima. Foi realizado estudo radiográfico no pré- e no pós-operatório para aferição dos ângulos de Böhler e de Gissane. Resultados O ângulo de Böhler médio dos casos antes da cirurgia foi de 2,5º, apresentando aumento da média dos ângulos para 25,3º após o tratamento cirúrgico minimamente invasivo. O ângulo de Gissane médio dos casos antes da cirurgia foi de 136,3º, apresentando diminuição da média dos ângulos para 114,3º na análise após a cirurgia. Conclusão A técnica cirúrgica minimamente invasiva permite melhora dos parâmetros radiográficos (ângulos de Böhler e Gissane) nas fraturas intra-articulares de calcâneo, com sua adequada recuperação da forma anatômica.

RESUMEN Introducción: en febrero de 2010 se introdujo la técnica colangiopancreatografía retrógrada endoscópica en la provincia de Matanzas, dando cobertura a los casos con esta indicación, y atendiendo a un importante grupo de pacientes en edad geriátrica. Objetivo: describir los resultados después de ocho años de trabajo. Material y Métodos: se realizó un estudio descriptivo, transversal, en pacientes mayores de 65 años, quienes se realizaron el estudio en el servicio de Cirugía de Mínimo Acceso del Hospital Universitario Clínico Quirúrgico "Comandante Faustino Pérez Hernández", en el período de enero 2010 a enero de 2018. La población de estudio quedó conformada por 698 pacientes, que cumplieron con los criterios de inclusión con 713 informes de colangiopancreatografía retrógrada endoscópica. Resultados: predominó el sexo femenino y la canulación del conducto deseado, se utilizó como técnica complementaria el pre corte, en cuanto a los diagnósticos realizados la litiasis coledociana resultó ser el más frecuente, y la colangiopancreatografía retrógrada endoscópica con acción terapéutica fue la más realizada con la pancreatitis como complicación más encontrada, fallecieron en el período estudiado cuatro pacientes como consecuencia de colangitis aguda grave. Conclusiones: los resultados obtenidos son similares a los realizados, en poblaciones sometidas al proceder en general, independientemente a la edad de los pacientes, resultando ser un proceder seguro en edades geriátricas, siendo la colangiopancreatografía retrógrada endoscópica en este medio un arma con gran utilidad diagnóstica y terapéutica con una frecuencia de morbi-mortalidad que lo garantiza (AU).


SUMMARY Introduction: the endoscopic retrograde cholangiopancreatography technique was introduced in the province of Matanzas in February 2010, for the treatment of the cases with this indication and attending an important group of patients in geriatric age. Objective: to describe the results of this technique after eight years of use. Method: a descriptive, cross-sectional study was carried out in patients aged more than 65 years, who were attended in the service of Minimal Access Surgery of the Clinical Surgical University Hospital "Comandante Faustino Pérez Hernández", in the period January 2010-January 2018. The studied population was 698 patients who fulfilled the inclusion criteria for 713 reports of endoscopic retrograde cholangiopancreatography. Results: female sex and cannulation of the desired duct predominated; pre-cut was used as complementary technique. Choledocholithiasis was the most frequent diagnosis; the most used therapeutic action was endoscopic retrograde cholangiopancreatography, and pancreatitis was the most commonly found complication. In the studied period, four patients died as a consequence of severe acute cholangitis. Conclusions: the obtained results are similar to those obtained in populations undergoing the procedure in general, patients´ age-independent. It is a safe procedure in geriatric ages. The endoscopic retrograde cholangiopancreatography is a tool of a great diagnosis and therapeutic usefulness in our midst, with a morbid-mortality frequency guarantying it (AU).

Abstract Objective: To compare intra- and post-operative consequences associated with Z-frenuloplasty and laser therapy for both upper labial and lingual frenulectomies. Material and Methods: Clinical data of 120 consecutive patients with a mean age of 11 years and 2 months (age range from 9 years and 1 month to 14 years and 3 months) with hypertrophic labial and lingual frenula were assembled. Of the 70 labial frenula, 35 were removed through Z-frenuloplasty (Group 1) and 35 with laser (Group 2); of 50 lingual frenula, instead, 25 were extracted through Z-frenuloplasty (Group 1A) and 25 with laser (Group 2A). The cutting device was Laser Diode Handy 10 in continuous mode. Finally, the time of the surgery, pain and swelling were measured 24-48 hours after the removal. VAS scale was used. Results: The time of the surgery, VAS score after the removal and the swelling were lesser in Group 2 and 2A. Conclusion: Both Z-frenuloplasty and Laser therapy are valid instruments to remove frenula. Moreover, laser offers more advantages like less use of anesthesia, no bleeding in the operating phase, no need for suturing, a faster tissue healing and minor limitations in speech and nutrition.

Introducción. La Lipoinyección de grasa autóloga con fines estéticos es una de las técnicas más jóvenes que permiten resolver problemas relacionados con valores concernientes a las elecciones personales que apuntan hacia la belleza, es un procedimiento quirúrgico sencillo y con resultados rápidos. Es una alternativa segura, real, sencilla y accesible que abarata costos, sobre todo con el uso de equipos y tecnología. Objetivos. Caracterizar los resultados estéticos de la Lipoinyección Labial con Grasa Autóloga en pacientes que asistieron a la consulta externa de Cirugía Plástica del Hospital Universitario "General Calixto García". Material y Método. Se realizó un estudio prospectivo, descriptivo, de corte transversal, en el período comprendido entre octubre de 2018 a septiembre del 2019. Se seleccionaron aquellos que cumplieron los criterios de inclusión y aceptaran a participar en el estudio voluntariamente. La muestra quedó constituida por 23 pacientes. La información obtenida se procesó mediante bases de datos confeccionadas en Excel, lo que permitió la realización de la estadística descriptiva simple para las variables individuales como media, desviación estándar y razón. La disposición a colaborar en el estudio se recogió en el modelo de Consentimiento Informado elaborado para esta investigación. Resultados. Predominio del sexo femenino en el 60.87 % de los operados. El grupo de edad entre 51 a 61 años fue el de mayor número de pacientes. Las complicaciones inmediatas más frecuentes en los labios fueron: la inflamación y los hematomas; las tardías fueron nodulaciones y endurecimiento. El 82.60 % de los pacientes se mostró satisfecho con los resultados. Conclusiones. La Lipoinyección labial con Grasa Autóloga se caracterizó por ser un método capaz de satisfacer el resultado estético. Las variables edad y sexo evidencian que las mujeres se realizaron más Cirugía Estética Labial que los hombres. Las variables clínicas labios finos y volumen evidencian que el aumento de la edad es una condición importante que se asocia a la demanda de esta técnica quirúrgica y que el ser hombre o mujer no establece una diferencia para la solitud de este tipo de técnica quirúrgica. Se presentaron complicaciones inmediatas y tardías. Los niveles de satisfacción expresados por los pacientes fueron altos.


Introduction. Autologous fat lipoinjection for aesthetic purposes is one of the youngest techniques that allow solving problems related to values concerning personal choices that point towards beauty, it is a simple surgical procedure with quick results. It is a safe, real, simple and accessible alternative that lowers costs, especially with the use of equipment and technology,Objectives. To characterize the aesthetic results of Lip Lipoinjectionwith Autologous Fat in patients who attended the Plastic Surgery outpatient clinic at the "General Calixto García" University Hospital. Material and Method. A prospective, descriptive, cross-sectional study was conducted in the period from October 2018 to September 2019. Those who met the inclusion criteria and voluntarily agreed to participate in the study were selected. The sample consisted of 23 patients. The information obtained was processed through databases prepared in Excel, which allowed the performance of simple descriptive statistics for individual variables such as mean, standard deviation and ratio. The willingness to collaborate in the study was included in the Informed Consent model prepared for this research. Results. female predominance in 60.87% of those operated on. The age group between 51 and 61 years was the one with the largest number of patients. The most frequent immediate complications on the lips were: inflammation and bruising; the later ones were nodulations and hardening. 82.60% of the patients were satisfied with the results.Conclusions. Lip Lipoinjection with Autologous Fat was characterized as a method capable of satisfying the aesthetic result. The age and sex variables show that women underwent more cosmetic lip surgery than men. The clinical variables thin lips and volume show that increasing age is ́t a condition that is associated with the demand for this technique and that the Being male or female does not make a difference to the solitude of this type of surgical technique. There were immediate and late complications. The satisfaction levels expressed by the patients were high.

BACKGROUND: We aimed to assess the dose needed to achieve the propofol effect-site concentration using target-controlled infusion in intellectually disabled patients and to detail the most effective method for achieving a safe level of consciousness without hemodynamic changes as well as detail any resulting adverse effects. METHODS: We performed a retrospective review of sedation service records of 138 intellectually disabled patients (51, mental retardation; 36, autism; 30, brain lesion, 12 genetic diseases, 9 dementia) aged over 15 years and weighing over 30 kg. These patients had received propofol via target-controlled infusion in the special care dental clinic of Seoul National University Dental Hospital from May 2008 to September 2018 for restorative treatment (112), minor surgery (13), prosthodontics (7), periodontics treatment (5), and implant (1). RESULTS: For all groups, the duration of dental treatments was 43 ± 18 minutes, total sedation time was 73 ± 23 minutes, and total BIS values was 57 ± 12. The propofol maintenance dosage values for each group were: mental retardation, 3 ± 0.5 (2–4) µg/ml; autism, 3.1 ± 0.7 (2–5) µg/ml; brain lesion, 2.8 ± 0.7 (1.5–5) µg/ml; genetic disease, 2.9 ± 0.9 (1–4) µg/ml; and dementia 2.3 ± 0.7 (1–3.4) µg/ml. CONCLUSIONS: The dementia group needed a lower dosage to reach a safe, effective propofol effect-site concentration than the other groups. Since there were no complications, deep sedation is a great alternative to general anesthesia for dental treatment of intellectually disabled patients.

ABSTRACT

The purpose of this study was to analyze outpatient referral patterns of children in Jeonbuk National University Dental Hospital. All patients under 16 years old who were consulted from the external facilities were reviewed based on the electronic medical record of Jeonbuk National University Dental Hospital from 2015 to 2018.Total 720 boys and 460 girls with an average age of 7.4 years were referred from local dental clinic (85.2%) for treatment severity (90.2%). 3.2% of patients has disability with the highest number of intellectual disabilities. The patients were usually referred at the pre-treatment stage with higher rates if they had disability or medical problem. Referral rate by chief complaints was highest in surgery, followed by reparative treatment and orthodontic treatment. 2(nd) referrals in pediatric dentistry had a higher rate of referrals to minor surgery and pulp treatment than of first referrals. The time point of 2(nd) referral was relatively delayed with increased ratio of ‘during treatment’.These results suggest necessities of appropriate case selection and referral based reasonable criteria for pediatric dentist.

Introduction : La pratique de l'anesthésie en médecine humanitaire est contraignante. Elle l'est davantage dans un pays à ressources limitées et à faible revenu en période de conflit. Objectif : Décrire le devenir des patients opérés en médecine humanitaire dans une zone de conflit à Niono au Mali. Méthodologie : Il s'agissait d'une étude prospective et descriptive sur une période 30 jours. La saisie et l'analyse ont été faites par Windows 8, Epi info 3.5.3.fr et le khi² pour le test statistique. Résultats : Au total, 63 patients étaient colligés pendant la période d'étude. La moyenne d'âge était de 33,3 ± 18, 5 ans et une prédominance du sexe féminin. Les urgences représentaient 65,1% des cas. L'indication opératoire était gynécologique ou obstétricale dans 63, 5% des cas, suivie de la chirurgie viscérale (20,6%), de l'urologie (11,1%) et la traumatologie dans 4,8% des cas. Les patients étaient ASA I dans 69,8% et de la classe Altemeier II dans 81%. Une antibioprophylaxie était instaurée dans 88,9% des cas. Une anesthésie générale a été faite dans 76,2% suivie de la rachianesthésie dans 11,1%. Un évènement per opératoire était observé dans 66,7% des cas. La survenue d'une complication infectieuse était notée dans 11, 1% des cas. Il s'agissait d'une suppuration pariétale dans 57,1%, un sepsis sévère, une fistule vésico-pariétale et un choc septique dans 14,3% chacun. La durée médiane d'hospitalisation était de 4, 19 jours. L'évolution était favorable dans 98,4%. Conclusion : Dans nos pays, le conflit armé influence la bonne organisation du système de santé avec une augmentation de la morbidité-mortalité

ABSTRACT

Se describe el caso clínico de una adolescente de 12 años de edad, quien había estado ingresada 5 años atrás en el Servicio de Terapia Intensiva del Hospital Pediátrico Docente Sur Antonio María Béguez César de Santiago de Cuba por presentar enfermedad estafilocócica y amenorrea primaria. Esta vez acudió a consulta con dolor abdominal recurrente desde hacía 3 meses, localizado en hipogastrio, acompañado de náuseas y vómitos, que se aliviaba con los analgésicos habituales. Se le realizó la prueba de embarazo cuyo resultado fue negativo y las imágenes ecográficas sugirieron la presencia de hematocolpos. Se le realizó himenotomía y egresó de la institución 48 horas después con evolución favorable.


The case report of a 12 years-old adolescent is described who had been admitted 5 years ago in the Intensive Therapy Service of Antonio María Béguez Caesar Southern Teaching Pediatric Hospital in Santiago de Cuba staphylococcal disease and primary amenorrhoea. This time she attended the service with recurrent abdominal pain for 3 months, located in hypogastrium, accompanied by nauseas and vomits that were alleviated with the habitual analgesic ones. The pregnancy test was carried out with negative result and the echographic images suggested hematocolpus. The himenotomy was practiced and was discharged from the institution 48 hours later with favorable clinical course.

ABSTRACT

Los barberos a través del tiempo, han dejado una estela para muchos, poco apreciada. El presente relato intenta dar un poco más de lustre a su nombre, observándolo sucintamente desde varias facetas: tocador de guitarra y cantador, navegante, artesano; intentando romper la creencia de que fueron personas incultas y sin conocimientos. También se realizará un acercamiento hacia los barberos que laboraron en nuestra Cuenca de los Andes desde el siglo XVI hasta su extinción en el siglo XX.


Barbers over time, have left a trail which is little appreciated. The present account tries to enhance its name, observing it briefly from several aspects: guitar player and singer, navigator and craftsman; trying to break the belief that they were uncultured and uneducated people. There will also be an approach to the barbers who worked in our city "Cuenca de los Andes" from the sixteenth century until its extinction in the twentieth century.

Introdução: Os pacientes com defeitos de parede abdominal chegam ao consultório do cirurgião plástico em situações muitas vezes complexas, necessitando de abordagem cirúrgica avançada. Métodos: Estudo primário, retrospectivo e descritivo de pacientes submetidos a procedimentos cirúrgicos de reconstrução de parede abdominal pelo Serviço de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE). Resultados: Foram revisados e incluídos os prontuários de 18 pacientes, 15 (83,3%) do sexo feminino e 3 (16,7%) do masculino, com idade variando de 16 a 79 anos (média de 41 anos). Dezessete pacientes possuíam histórico de cirurgia prévia (94,4%), sendo a cesárea presente em 8 dos casos (44,4%), seguida de cirurgia oncológica com 6 (33,3%), cirurgia do trauma com 2 (11,1%) e bariátrica com 2 (11,1%). Em relação à etiologia do defeito, 8 (44,4%) eram decorrentes de fasciite necrosante, 4 (22,2%) de hérnia incisional, 2 (11,1%) por trauma, 2 (11,1%) por infecção de ferida operatória e 2 (11,1%) por neoplasia de parede abdominal, sendo somente um (5,5%) paciente com defeito de espessura total. A técnica cirúrgica de separação dos componentes foi realizada em 7 dos casos (38,9%), seguida de retalho de avanço simples em 6 (33,3%), fechamento com tela associado à abdominoplastia em 3 (16,7%), e expansor tecidual em 2 (11,1%). Quanto às complicações, houve 4 casos (22,2%). Conclusões: Defeitos de parede abdominal são casos desafiadores para o cirurgião plástico, seu tratamento se mostra árduo, porém com resultados satisfatórios mesmo nos casos mais severos.


Introduction: Patients with abdominal wall defects present challenging complications that require the use of advanced surgical approaches. Methods: This primary, retrospective, and descriptive study evaluated patients who underwent abdominal wall reconstruction at the Plastic Surgery Service of the Clinics Hospital of the Federal University of Pernambuco. Results: The medical records of 18 patients were reviewed, including 15 women (83.3%) and 3 men (16.7%), with a mean age of 41 years (range, 16-79 years). Seventeen patients (94.4%) had a history of previous surgery. The causes of abdominal injury were cesarean section in eight cases (44.4%), oncologic surgery in six (33.3%), trauma surgery in two (11.1%), and bariatric surgery in two (11.1%). The etiology of the defect was necrotizing fasciitis in eight cases (44.4%), incisional hernia in four (22.2%), trauma in two (11.1%), surgical wound dehiscence in two (11.1%), abdominal wall neoplasia in two (11.1%), and total thickness defect in one (5.5%). The surgical interventions included the component separation technique in seven cases (38.9%), simple VY advancement flap in six (33.3%), closure with abdominoplasty in three (16.7%), and tissue expander in two (11.1%). Four patients (22.2%) presented complications. Conclusions: Abdominal wall defects are challenging cases for plastic surgeons, as their treatment is difficult, but the results are satisfactory even in the most severe cases.

Introdução: Primeira colocada entre as mais procuradas cirurgias plásticas genitais, a ninfoplastia ou labioplastia visa a correção da hipertrofia dos pequenos lábios vaginais e prepúcio, retirando seu excesso, sem interferir na sua função de proteção da vagina e auxílio na lubrificação genital. Diversos tipos de classificações foram propostos para facilitar a compreensão do grau de hipertrofia dos pequenos lábios vaginais e ajudar na escolha da técnica da labioplastia. Após analisar várias classificações, o autor propõe uma nova classificação, no intuito de facilitar a compreensão da hipertrofia das ninfas, capuz do clitóris e prepúcio e ajudar na escolha da técnica apropriada para labioplastia. Métodos: Foi feita uma busca na literatura médica PubMed/Medline com os termos hipertrofia lábios vaginais, labioplastia, labiaplasty, labioplasty, lábia minora hipertrophy, labial protrusion. Foram analisadas todas as classificações descritas nos trabalhos encontrados. Resultados: Uma nova classificação foi proposta. A hipertrofia dos pequenos lábios vaginais foi classificada em 4 graus: Grau 0 (≤ 1 cm), Grau 1 (> 1 cm e ≤ 3 cm), Grau 2 (> 3 cm e ≤ 5 cm) e Grau 3 (> 5 cm). Conclusões: A nova classificação, além de facilitar a compreensão do tamanho e extensão da hipertrofia das ninfas, também auxilia na escolha da técnica a ser escolhida para a labioplastia.


Introduction: Nymphoplasty or labioplasty is the most common genital plastic surgery. The objective of labioplasty is to correct hypertrophy of the labia minora and clitoral prepuce, removing excess tissue without affecting their function of protecting the vagina and aiding in genital lubrication. Several types of classifications have been proposed to facilitate the understanding of the degree of hypertrophy of the labia minora and assist in selecting the most suitable procedure in labioplasty. After analyzing several classifications, the author proposes a new classification to facilitate the understanding of hypertrophy of the labia minora, clitoral hood, and vaginal prepuce and help select the best labioplasty procedure. Methods: A literature search was conducted in PubMed/Medline using the following terms: hipertrofia lábios vaginais, labioplastia, labiaplasty, labioplasty, labia minora hypertrophy, and labial protrusion. All the classifications described in the identified studies were analyzed. Results: A new classification has been proposed. Hypertrophy of the labia minora was classified in four grades: grade 0 (≤ 1 cm), grade 1 (> 1 cm and ≤ 3 cm), grade 2 (> 3 cm and ≤ 5 cm), and grade 3 (> 5 cm). Conclusions: The new classification improves the understanding of the size and extent of hypertrophy of the labia minora and helps select the best procedure in labioplasty.

ABSTRACT

Chemodenervation with botulinum toxin (BTX) has been recommended for focal spasticity. BTX injection should be performed with caution in patients with bleeding disorders and/or receiving anticoagulation therapy. We present a case of BTX injection for post-stroke spasticity in a patient with hemophilia A who could not take oral spasmolytics due to chronic hepatitis C. To minimize the bleeding risk, we replaced factor VIII intravenously in accordance with the World Federation of Hemophilia guidelines for minor surgery. FVIII (3,000 IU) was administered 15 minutes before BTX injection. One day later, 2,000 IU was administered, and 2 days later, another 2,000 IU was administered. We performed the real-time Ultrasound-guided BTX injection three times, then spasticity and upper extremity function improved without adverse events. BTX injection can be considered as a treatment option for spasticity among patients with hemophilia.

ABSTRACT

Oral anticoagulant therapy is frequently and increasingly prescribed for patients at risk of arterial or venous thromboembolism (VTE). Although elective surgical or invasive procedures have necessitated temporary interruption of anticoagulants, managing these patients has been performed empirically and been poorly investigated. This study was designed to evaluate the adequacy of perioperative anticoagulation using enoxaparin. This was a retrospective, single-center study that evaluated the efficacy and safety of therapeutic-dose enoxaparin for bridging therapy in patients on long-term warfarin at Soonchunhyang University Hospital in Korea between August 2009 and July 2011. Warfarin was discontinued 5 days before surgery, and enoxaparin was administered twice daily by subcutaneous injection at a dose of 1 mg per kg from 3 days before the procedure to the last dose 24 hours before the procedure. Anticoagulation was restarted if proper hemostasis had been confirmed. There were 49 patients, of whom 25 (51%) were men, and the mean age was 63 years. Thirty-four (69%) received warfarin therapy for VTE, and 9 (18%) for atrial fibrillation. Twenty-nine patients (59%) underwent major surgery and 20 (41%) minor surgery. The mean postoperative duration of enoxaparin was 4 days. No patients had thromboembolic complications through 30 days after the procedure. The overall 30-day mortality rate was 0%. In conclusion, our findings demonstrate that bridging therapy with therapeutic-dose enoxaparin is feasible and associated with a low incidence of major bleeding and no thromboembolic complications. However, the optimal approach to managing patients perioperatively is uncertain and requires further evaluation.

ABSTRACT

BACKGROUND: Anxiety control remains an important concern in dental practice. We evaluated the incidence, nature, and sequelae of complications during and after minor oral surgeries performed under intravenous midazolam and fentanyl sedation using the titration technique. METHODS: The medical records of patients who had undergone minor oral surgeries under moderate intravenous midazolam and fentanyl sedation at our institution between January 1, 2015 and December 31, 2015 were retrospectively evaluated. Age, sex, body mass index, medical history, American Society of Anesthesiologists (ASA) classification, indications for sedation, amount of sedative used, surgical duration, and recovery time were evaluated for all patients. RESULTS: In total, 107 patients aged 9–84 years were included. ASA class I and class II were observed for 56.1% and 43.9% patients, respectively. Complications associated with sedation occurred in 11 (10.2%) patients. There were no serious adverse events. Oxygen saturation reached 95% during the procedure in six patients; this was successfully managed by stimulating the patients to take a deep breath. Two patients exhibited deep sedation and one exhibited paradoxical excitement. After the procedure, one patient experienced nausea without vomiting and one exhibited a prolonged recovery time. The surgical procedures were completed in all patients. Obesity was found to be significantly associated with sedation-related complications. CONCLUSIONS: Our results suggest that complications associated with intravenous midazolam and fentanyl sedation using the titration technique for minor oral surgeries are mostly minor and can be successfully managed with no prolonged sequelae.

ABSTRACT

Aseptic techniques are those practices designed to reduce the risk of surgical site infection (SSI), defined as such, all those that occur within the first 30 days of the procedure. While the patient’s risk of developing an SSI in dermatologic surgery is low since many of the procedures are considered sterile, there are different factors associated with an increased or decreased risk of developing SSI. The characteristics of the surgical wound (such as involving infected or inflamed tissue or when breaks in the aseptic technique occur), patient characteristics (such as age, comorbidities, medication use and smoking) and procedure factors (such as setting, surgical technique, type of procedure, duration and body region involved). In this article we discuss the management of potential sources of infections such as personnel (hand washing, dressing), preparation of the patient, maintenance of a clean surgical environment, sterilization and the use of antiseptic solutions. Similarly, the current indications for prophylactic antibiotics for these procedures are considered.
